BEFORE YOUR VISIT
 

To help us serve you faster, please prepare the following:

Bring these documents:
 

  • Valid ID

  • PhilHealth ID and Member Data Record (if applicable)

  • HMO or insurance card (if applicable)

  • Previous laboratory results or medical records

  • Referral form (if any)
     

For laboratory tests:
Some tests require fasting for 8–12 hours. Please confirm instructions when scheduling or registering.


UPON ARRIVAL
 

  1. Proceed to the Reception / Registration Area

  2. Present your documents

  3. Secure your queue number

  4. Wait to be called for triage or consultation


Our staff will guide you through every step of the process.


PATIENT GUIDELINES

To maintain safety and order inside the hospital:
 

  • Follow instructions of doctors and nurses

  • Wear a face mask if required by infection control policy

  • Keep valuables secured

  • Maintain cleanliness in all areas

  • Avoid loud noise and disturbance

VISITOR GUIDELINES

If admission is required, please prepare:

  • Comfortable clothing

  • Maintenance medications (if instructed by doctor)

  • Important documents and IDs
     

Our admission staff will assist you with room assignment and billing procedures.


BILLING & PAYMENT

The billing section will guide you regarding:

  • PhilHealth deductions

  • HMO processing

  • Payment options

  • Official receipts
     

Please settle balances before discharge unless otherwise advised.


DISCHARGE PROCESS

Before leaving the hospital, ensure you have:
